New Patient Information
Your First Visit
When you come in for your first visit with us, the first thing you’ll see will be a smiling face. Our front desk team will greet you, offer you a beverage (water, coffee, or tea), and then escort you to a treatment room. You’ll then get to meet Dr. James R. Nelson, and he’ll ask you some questions about your previous dental experiences, as well as what you hope to accomplish by coming to Nelson Dental Care.
At this point, some photos and x-rays will be taken of your teeth, and then Dr. Nelson will perform a comprehensive oral exam. After this, Dr. Nelson will go over his findings with you and recommend a treatment plan that is specifically tailored to your particular dental needs. Once you’ve had a chance to ask any questions you might have, you’ll be escorted to the front desk where we’ll help you schedule your next appointment.

These forms will help us get to know you just a little bit better by providing us with basic information. To save a little time at your appointment, you can choose to complete them beforehand. Simply print them, fill them out, and bring them to our office to help things fly by even faster!
